Patient Forms & Financing
Before you undergo any type of plastic surgery at our Idaho Falls practice, our staff will ask you to fill out some paperwork and make decisions about payment. We want your experience at Freeman Plastic Surgery to be enjoyable and efficient from start to finish, so we’ve added some of these patient resources to our website that you can fill out in advance in the comfort of your home.
People travel to see Dr. Freeman for plastic surgery from Pocatello, Twin Falls, and other Idaho cities, as well as throughout Wyoming, Utah, and Canada. If you have questions about our policies, request a consultation online or call our office at (208) 881-5351 to schedule an appointment today.
To help you prepare for your consultation and procedure, we’ve provided some of our patient forms. You may print and fill out these forms at home in advance to help save time during your first appointment.

Non-Surgical Patient Forms
Patients seeing Freeman Plastic Surgery for services like Botox®, fillers, and laser treatments are encouraged to complete the following Check-In form before coming in for your treatment to give your providers a complete picture of your needs, and to save you time during your appointment.
At Freeman Plastic Surgery, we strive to stay competitive with our costs so that all our patients find the procedures they desire within logistical reach. Regardless, price is not the best way to choose a plastic surgery practice. As one of Dr. Freeman’s mentors often said, “There are 2 things to never shop for on price: parachutes and plastic surgery.” Unfortunately, if a cheap surgery is not performed well, the second surgery to correct complications often costs 2 to 3 times more than the original procedure.
Our prices are based on national averages released each year by the American Society for Aesthetic Plastic Surgery. While we could easily charge more based on our experience, we intentionally attempt to keep our prices around the national average to allow everyone the ability to afford quality plastic surgery. We believe in keeping our costs as low as possible while still adhering to strict standards of excellence and safety. You’ll never see us offering sales on surgical procedures, but we do cut costs whenever possible so that our patients have access to true value.
We understand that paying for plastic surgery in full isn’t always possible for many people. For those who would prefer to pay in installments, we’re pleased to offer financing through the following reputable lenders:
- PatientFi is the friendly way to pay for elective treatments and procedures. With PatientFi, you can get what you want today, and pay for it over time on a friendly monthly plan. Applying only takes one minute, and the best part? We use a soft credit check so you can apply and check your offer worry-free with no impact to your credit.
